A Medicare Advantage Special Needs Plan (SNP) is a specialized Medicare Advantage plan for people with specific health or financial situations. Rather than a general-purpose plan, an SNP is built around the needs of a specific group of beneficiaries.

Types of Special Needs Plans

There are three main types of SNPs. A Chronic Condition SNP (C-SNP) serves people with qualifying chronic conditions, such as diabetes or heart disease. A Dual Eligible SNP (D-SNP) serves people who qualify for both Medicare and Medicaid. An Institutional SNP (I-SNP) serves people who meet institutional care criteria, such as living in a long-term care facility.

What Makes SNPs Different

SNPs are designed around the population they serve, so they often include care coordination, disease management programs, and provider networks tailored to those specific needs — features a general Medicare Advantage plan may not emphasize in the same way.

Who Qualifies

Qualification depends on the SNP type: a qualifying diagnosis for a C-SNP, dual Medicare and Medicaid eligibility for a D-SNP, or meeting institutional care requirements for an I-SNP. A licensed agent can help confirm which, if any, SNP type you may qualify for.

Frequently Asked Questions

Eligibility depends on the SNP type: C-SNPs require a qualifying chronic condition, D-SNPs require dual eligibility for Medicare and Medicaid, and I-SNPs require meeting institutional care criteria.
